Why is the frame slightly off the time I asked for?
Because the seek happens before decoding, which is what makes it fast: the engine jumps to the nearest keyframe at or before your timestamp instead of decoding everything up to that point. On footage with widely spaced keyframes the frame you get can be a second or two early, so ask for a slightly later time when you need a specific moment.
What quality is the image?
A high-quality JPEG at the video’s own resolution — the frame is not scaled down. Resize the video first if you want a smaller still.
